What Is Dental Bone Grafting

Dental bone grafting is a surgical procedure that replaces or augments deficient bone in the jaw to create a sufficient foundation for dental implant placement. When a tooth is lost, the jawbone in that area no longer receives stimulation from chewing forces, causing it to gradually resorb (shrink) over time. Studies indicate that as much as forty to sixty percent of bone width can be lost in the first two to three years following tooth extraction.

An implant requires a specific minimum bone volume typically at least six to seven millimetres in width and ten millimetres in height for stable placement and osseointegration (the process by which the implant fuses with the bone). Autograft (Your Own Bone)

Bone harvested from another area of the patient’s own body most commonly the chin, the jaw behind the wisdom tooth area, or the hip in larger cases. Autografts offer the highest success rates as the material is biologically identical to the recipient site. The drawback is the requirement for a second surgical site.

Allograft (Human Donor Bone)

Processed bone from a human tissue bank, which has been sterilised and treated to eliminate any infectious risk. Allografts are the most commonly used graft material in dental practice, eliminating the need for a donor site on the patient.

Xenograft (Animal-Derived Bone)

Most commonly bovine (cow) derived, xenograft materials are processed into a mineral scaffold that the patient’s own bone cells gradually replace over time. These materials have excellent clinical track records and are widely used in Chennai dental practices.

Synthetic Bone Substitutes

Calcium phosphate ceramics and other synthetic materials mimic the mineral component of natural bone and provide a scaffold for bone regeneration without any biological sourcing concerns.

Do You Actually Need a Bone Graft for Dental Implants

Not every implant patient requires bone grafting. Patients who have their implant placed shortly after tooth extraction (immediate or early implant placement) often have sufficient bone volume for implant placement without augmentation. Those who lost teeth years or decades ago are far more likely to require grafting.

A three-dimensional CBCT (cone beam computed tomography) scan is the definitive way to assess bone volume and quality at the intended implant site. This imaging provides precise measurements and eliminates guesswork about whether grafting is needed.

Bone Grafting Procedures What They Involve

Socket Preservation Graft

Performed at the time of tooth extraction, graft material is placed directly into the empty socket to minimise bone loss during healing. This is the simplest grafting procedure and is the most effective way to prevent the need for more complex grafting later.

Ridge Augmentation

For areas where significant bone loss has already occurred, graft material is placed along the surface of the jawbone and covered with a collagen membrane to guide bone regeneration. Healing takes three to six months before implant placement can proceed.

Sinus Lift

Specific to the upper back jaw, where the sinus cavity sits close to the crest of the jawbone. The sinus floor is carefully elevated and graft material placed beneath it to create height for implant placement. This is the most complex routine grafting procedure.
